How do I become a patient for comprehensive care in the undergraduate program?
New patient appointments for initial examinations for our undergraduate program are made by calling (206) 616-6996. Appointments are made throughout the year based on the need for new patients.
What should I bring to my first appointment?
Your first stop will be room B-307 (directions), the Dental Patient Registration Office. You will need the following information in order to register:
- Photo ID
- Any dental insurance information
- X-rays taken within the past 12 months
- Medical coupon for DSHS (if applicable)
- Name of guardian and/or power of attorney (for children)
- If you do not speak English and need the services of an interpreter to communicate with our staff, we encourage you to bring someone to assist you.

What can I expect at the new patient examination appointment?
At this initial appointment you will be required to complete a patient registration packet. If you are unable to fill out these forms, you must bring someone with you that can.
Your first appointment will consist of a comprehensive assessment of your teeth and mouth. A complete set of dental x-rays will be taken. You should allow three to four hours for this visit. No dental treatment is provided during this exam. There is a fee of $175 for the exam and x-rays. Insurance and select medical coupons can be used to pay for this visit.
If you are not accepted for care in our undergraduate student clinics, you may be referred to faculty practice clinics or a graduate specialty clinic
What happens after I'm accepted for treatment in a dental student clinic?
Your first appointment will consist of a comprehensive assessment of your teeth and mouth. A complete set of dental x-rays will be taken. You should allow three to four hours for this visit. Who will provide my dental care?
Dental services are performed by students under the direct supervision of experienced dentists who are faculty members of the School of Dentistry. Because a faculty member will supervise and evaluate each step of the care provided by your student dentist, services will take longer than at a private dental office. This attention to detail requires more of your time, but assures you of quality dental care.
How long will it take to complete my treatment?
Due to the instruction time during your appointment, patient visits are two to three hours long. You should be available for appointments at least twice a month until your treatment is completed.
It may take more than a year to complete a long treatment plan. If it will be difficult for you to meet this appointment schedule, you should consider seeking care from a faculty dentist or a provider in the community.
Our fees are generally lower than those of a private dentist. We offer these lower fees in recognition of the extra time needed to be treated by a dental student. Payment for treatment is due at the time of your appointment. We do not offer payment programs or a "sliding fee scale."
Your student dentist will explain the proposed treatment and estimated fees. Normally, we adjust our fees annually as costs increase.
How do I pay for my dental care?
Patients are required to pay for their treatment at each appointment. We accept cash, check, debit or credit cards (MasterCard or Visa) and offer the Citi Health Card as a payment option.
The Citi Health Card offers no interest payment plans, low monthly payments and an easy application process that allows for treatment to begin immediately upon credit approval. Applications for the Citi Health Card can be done in our offices or at home. To apply at home by phone call 1-866-843-2330, to apply using the internet go to http://www.citibank.com/us/cards/cardserv/healthcrd/.
Our Patient Financial Office in room B-232 will assist in answering questions and preparing claims for those patients with dental insurance or Medicaid options. For information please call (206) 543-5297.
The School of Dentistry is NOT able to provide child care services for patients while they are receiving care. Children cannot be left unattended in the reception areas during a parent's appointment. Also, children are not allowed in any clinic treatment area with the exception of the Pediatric Clinic for their own dental care.
PLEASE NOTE: pets are not allowed in the building
What if I need to miss an appointment?
Since students need to deliver patient care in order to graduate, it is particularly important that you keep your scheduled appointments. We ask that patients come to all appointments on time as scheduled. If you need to change your appointment in an emergency, please call your assigned dental provider as soon as possible.
There is a $30 charge for appointments cancelled with less than 24 hours notice.
Can I be treated by a faculty dentist?
Yes. If you are interested in receiving care from one of our faculty dentists, private dental care is provided in our faculty clinics (contact information). The fees are similar to those of private dentists in the community.
Each department has individual requirements regarding acceptance for care. In some cases you may need x-rays and a referral from your private dentist. For additional information, please contact the department directly:
