Immunization Requirements

Employee Responsibility

All faculty, students, staff, and visitors/observers who work in laboratory or  clinical environments which may expose healthcare workers to either blood borne or airborne pathogens, are required to be immunized and/or screened as listed below.

  • Tuberculosis
    A negative tuberculin skin test within the year or appropriate TB screening and Treatment.

  • Tetanus-diphtheria
    Within the past 10 years.

  • Poliomyelitis
    A series of three Sabin (oral) polio, including booster after the age of four years.

  • Measles
    Measles (Rubeola) vaccine without Immune Globulin after 1967 (2 doses after months of age and at least one Month apart. One of these should be received after 1980 because of weaknesses in earlier vaccines).

  • Rubella
    Immunization or positive titer in your lifetime. You are expected to provide proof of one or the other.

  • Mumps
    Immunization or Disease.

  • Hepatitis B.
    Immunization is required by means of a series of 3 injections, the second one month after the first and the last five months after the second.

  • Varicella (Chicken Pox)
    History of disease or immunization when available.

Authenticated information must be submitted and/or immunization and screening obtained for clearance on this requirement. Authenticated information is either an immunization record signed by medical authority, international immunization record, or other documentation which certifies immunization status.
