Powerpoint slides for Graduation ceremony: submit 2 - 4 digital photos of you (fine to include family, friends, activities, etc.) that will be projected as you cross the stage at the ceremony. Email your photos no later than May 11 to Garret Yamaguchi, coordinator for the slide presentation during the ceremony.

GRADUATION SCRIPT

Deadline: May 5

The information that you provide for your script will be read as you walk across the stage to receive your hood. Complete the Script Turn-In Form online (Links located below forms header near the top of this page). Any Senior awards that you receive will be added to your script prior to the ceremony.

GRADUATION APPAREL ONLINE ORDERS

May 5 - May 25

Apparel Order

All graduates attending the Hooding ceremony are required to rent and wear graduation apparel. You can order your apparel online. Know your height in feet and inches and your weight in pounds. Mortarboards are one size fits all.

Be sure to select our ceremony (Dentistry Hooding) before placing your order. Total cost for Professional DDS cap, gown and hood rental is $55.95.

APPAREL PICK UP

Deadline: June 4

The School of Dentistry's assigned dates for apparel pick up are Monday - Wednesday (June 2-4) at the HUB from 11:00 am to 6:00 pm. If you are unable to go on the assigned date you can have a friend or family member pick up your apparel as long as they have a copy of your receipt.

We advise that you try on your apparel at the time you pick it up - you are responsible for making any changes prior to the ceremony. Please note that the apparel vendor removes the hoods from your packet for our office to pick up before the ceremony, so your packet will contain gown, cap and tassel only. Student services will pick up and transport all hoods to Meany Hall before the Ceremony.

GRADUATION BRIEFING SESSION

A briefing session for your class will be held to describe the ceremony and provide more details about graduation. Attendance is mandatory.

HONORS AND AWARDS CEREMONY

June 6
Hogness Auditorium
1:00 - 3:00 pm

Scholarships and awards will be presented to students. Each class also presents faculty and staff awards at this time. Class and clinic during this time will be cancelled so that the entire dental school community may attend. Seniors who have been chosen to receive awards will be notified prior to the ceremony. Your family and friends are welcome to attend the Honors and Awards Ceremony.

GRADUATION DAY PHOTOS

June 7
Red Square
12:30 pm

Graduates need to arrive at Red Square (in front of Gerberdeing Hall) no later than 12:30 pm for class photos. The photographer from PictureMan, Inc. will be there with photo order cards. Once the group photo is done (by 1:15 ), students should enter Meany by the front door (all other doors will be locked), proceed to the lower lobby for line-up which begins promptly at 1:30 pm. An outdoor class photo, and indoor photos of each senior getting their hood and shaking the Dean's hand will be taken and available for purchase by the Picture Man studio. At the time of your outdoor photo, the photographer will provide order forms and more information.

HOODING CEREMONY

June 7

2:00 pm

The School of Dentistry's graduation is called the Investiture of Doctoral Hoods or "Hooding." You are welcome to invite your family and friends to the ceremony and the reception following which will be held in the lobby of Meany Hall. Tickets are not required to attend our ceremony and there is no charge for parking on campus after 12:00 pm.

PUBLIC ENTRANCE

The front (main doors in front of By George) of Meany Hall is the public entrance. There is an elevator from the parking garage just outside the lobby. The main doors will open at 1:00 pm for public seating.

DISABILITY ACCOMMODATIONS

If one of your guests needs disability accommodations at our graduation ceremony (i.e special seating or sign language interpreter), please let us know by May 20 so we can coordinate with Disability Services. They ask for ten days advance notice.

GUESTS

There is no limit to the number of guests you may invite. This event does not require tickets.

CHILDREN/BABIES

Meany Hall has a room set aside for this; just ask one of the ushers. There's a video camera in the room so none of the action has to be missed!

RECEPTION

A reception is held directly after the ceremony in the Meany Hall lobby. The reception is usually over around 5:00 p.m., so feel free to stay as long as you wish.

POST-GRADUATION

DIPLOMA

Diplomas are mailed to the student's address selected on MyUW (local or permanent) three to four months following graduation. The diploma will list your name, degree, and any applicable honors.

SoD NOTE: We ask the Registrar's Office to add "with honors" to students' transcripts that graduated in the top 10% of their class - this won't show up for a few months after graduation, as it takes that long for all spring grades to be posted and the final class ranking to be calculated.

Diploma Name

Diplomas are issued with your name as it appears on MyUW and your transcript. It must include the student's legal first and last name. To indicate hyphens, spaces, lower/upper case letters, accents, or other special characters on your diploma, complete the Diploma Name Request Form. Submit it to the Graduation and Academic Records Office, 264 Schmitz Hall, by the last day of the quarter you plan to graduate.

To change your name on your University records, file a Change of Name form with the appropriate legal documentation which may include: marriage license/certificate, court order, divorce decree, passport or permanent visa. Forms are available at a registration window, 225 Schmitz Hall. See Name Change Policy for additional information.

To order additional or replacement diplomas, download the Diploma Replacement Form or contact the Office of Graduations and Academic Records at (206) 543-1803. The diploma replacement fee is $20.00.

The Registrar will mail your diploma to your permanent address approximately 3 months after graduation. To change or verify your address with the Registrar's office, call (206) 543-3868 during business hours or visit My UW.

COMMENCEMENT EXERCISES

June 14

The University's commencement exercises take place on Saturday, June 9 at 1:30 pm in Husky Stadium. You may participate in this ceremony as well, but you must make your own arrangements to do so. Graduates who request tickets to Commencement are required to participate in the ceremony! You must register for tickets and parking permits to attend commencement.

APPAREL RETURN

Deadline: June 14

Graduation apparel can be dropped off at the HUB until 6:00 pm after our ceremony on Saturday, June 7. Students who wish to drop their apparel at a later time are responsible for returning your gown and hood to the HUB. A hold will be placed on the transcripts and diploma of those who do not comply. The cap and tassel are yours to keep.

GRADUATION EXIT FORM

Students who are not cleared in all departments and areas will not receive their diploma until all obligations are met. You must be cleared from various commitments made during dental school, such as gold purchased, etc. Your locker, clinic unit, and mailbox need to be cleaned out before you leave. You will need to obtain signatures on the graduation checklist for each designated area. Turn in the completed, signed checklist to the Office of Student Services, Admissions, and Outreach in D323.

UW E-MAIL FORWARDING

Your UW e-mail will expire one quarter after your last class ends. All UW Alumni are eligible to to sign-up for a MyUW.net account. A MyUW.net account allows you to keep your current UW e-mail by forwarding it to a personal e-mail account. We highly encourage you to sign-up for MyUW.net. It would allow the School of Dentistry and your peers to keep in contact with you in the future. Links for this can be found on your MyUW homepage.

WASHINGTON STATE LICENSURE APPLICATIONS

Licensure Information

OFFICIAL GRADUATION DATE

Although the School of Dentistry Hooding ceremony takes place on Saturday, June 7, your official graduation date is June 13, 2008. It is important to always use this date on official documents. Your diploma will state that you received your degree on June 13, 2008.

DDS DEGREE/DIPLOMA

Assuming all of your grades are submitted, your DDS degree will be posted on your transcript by the end of June. You should receive your physical diploma four months after you graduate. If you need an interim document stating when you graduated, or if you have any further questions, please contact the Office of Student Services, Admissions and Outreach.
