Graduate Program Application Procedures

All application materials MUST be received by the appropriate offices at the University of Washington before September 1, 2008 for consideration in the class beginning Summer Quarter, 2009.

Please send all application materials (Questionnaire to Applicants, 3 Letters of Reference, Class Standing, and National Board scores) to the following address:

Follow these instructions carefully.

Sending application materials to the wrong address will cause a delay in the processing of your application.

1. Application for Admission to the Graduate School must be completed on-line here - the application fee is $50 and is payable only by credit or debit card.

• Graduates of dental schools outside the U.S.: request the registrars of all collegiate institutions you have attended to send two sets of OFFICIAL TRANSCRIPTS, one set directly to the Office of Graduate Admissions:

• United States graduates send only one set of official transcripts to the department address listed above.

2. Questionnaire to Applicants should be sent to the department.

3. Letters of Recommendation: Read the waiver statement at the top of the Evaluation forms and sign your name. Give a signed copy of this form to three professional references to fill out and have these individuals submit the form directly to the Counseling Services Coordinator. If you wish to submit additional letters of recommendation, you may do so.

4. Class Standing: Send the form to the Dean of your dental school for completion after signing the release statement on the reverse side.

5. Official National Board scores should be sent to the department.

6. Applicants that graduated from dental schools outside the U.S. must submit scores from the Graduate Record Examination (GRE). These scores must be received prior to the September 1st deadline. International applicants (except those from English-speaking countries) must also submit TOEFL scores (above 600 for the written test/above 237 for the online test), and ALL international applicants must submit a financial statement prior to the deadline. The GRE and TOEFL examination can be taken online.

Tuition

Estimate of tuition fees and expenses for a student enrolled full time for the 2007/2008 academic year are as follows:

• In-state tuition $ 12,050
• Out-of-state tuition $ 27,814 *
• Instruments per year $ 1500
• Books $ 1,280

*Out-of-state students have generally been able to satisfy residency requirements and be considered for in-state tuition by Autumn Quarter of their second year.
