Photo & Video Releases
There has been increasing growth in Internet photography and videography. Web sites such as My Space, Google, Yahoo, YouTube and numerous others have provided a platform for individuals to visually share information over the Internet. This information can also contribute in powerful ways to the public’s perception and opinion of an organization. Visual information has the potential to affect, both positively and negatively, the credibility of an organization, its clients and the community.
Out of respect for UW School of Dentistry patients, students and employees, we ask for your continued compliance with the following:
- Not taking or using photographs and videos of students or staff without their expressed consent.
- For privacy purposes, photos or videos of patients are only permitted for treatment purposes without a specific written authorization.
Please use these forms:
UW Faculty, Staff & Student Photo Release (one time waiver) for photos/video of workforce (faculty, students and staff). This is a one time waiver for miscellaneous purposes.
UW Faculty, Staff & Student Photo Release (for publications) for photos/video of workforce (faculty, students and staff). Use this to authorize the use of photos in a specific publication(s).
patient photo/video release (Privacy Policy 08, Attachment G) for photos/videos of patients.
